Memorial Brick and Statue at Humane Society of Utah
The Humane Society of Utah's Memorial Plaza is a place of tribute in memory of loved ones who have touched our lives—both animal and human. 
 
You can leave a permanent mark for someone special by purchasing a tax-deductible engraved brick installed in our Memorial Plaza. Personalize your brick with a message and graphic in the form below.
 
Bricks are a touching way to honor the passing of a pet, friend, or family member. Bricks can also be beautiful gifts for birthdays, anniversaries, and holidays. We can provide Brick Certificates after purchase to allow a gift recipient to choose their personalized message.
 
"In Memory of" and "In Honor of" dedications, pet names, people names, special messages and tributes, and birth/death dates are acceptable. Business names and advertising are not allowed. All messages are subject to approval by the Humane Society of Utah.
 
Bricks are installed twice a year, in April and October.

Brick Order Form
If purchasing a Large Brick, enter text:
Size: 6x9 inches Optional text is limited to three lines on a large brick. Each line can accommodate 16-18 characters (including spaces and punctuation).
If purchasing a Small Brick, enter text:
Size: 6x6 inches Optional text is limited to three lines on a small brick. Each line can accommodate 14-16 (including spaces and punctuation).
Graphic Options (for either brick size):

Graphic options include: Dog paw, cat paw, or heart. Please note that including a graphic will remove the space for approximately half of one line of text.
